Midwives Self Referral Form (Pregnancy referral for maternity booking)
Once you have found out that you are pregnant, please get in touch with a maternity team as soon as possible to book your first appointment.
Refer yourself to the The Princess Alexandra Hospital (and community midwifery teams) maternity services
Refer yourself to BHR University Hospitals. Between 8 to 10 weeks of pregnancy, you can apply to have a test for Sickle cell & Thalassaemia to eligible groups. Please email bhr-tr.newbornscreening@nhs.net or telephone Monday to Friday 8.00am to 4.30pm, Tel no 01708 503516.
